Resumen


La determinación del colesterol de lipoproteínas de baja densidad (LDL-C) es de gran importancia en el laboratorio clínico de rutina, ya que las guías internacionales de manejo de pacientes con riesgo cardiovascular hacen énfasis en su utilización como criterio de clasificación diagnóstica y también su disminución como objetivo terapéutico del tratamiento con estatinas o similares.

En la actualidad la determinación del LDL-C en la mayoría de laboratorios clínicos en Costa Rica se realiza mediante la estimación con la fórmula de Friedewald publicada en el año 1972, la cual se sigue utilizando ampliamente a pesar de conocer las limitaciones de su uso. Por lo tanto, se sugiere la necesidad de implementar métodos homogéneos directos para la determinación de LDL-C.

La implementación de métodos directos para la determinación de colesterol LDL ha ido en incremento en los últimos años; sin embargo, permanecen algunas dudas sobre la heterogeneidad de los resultados según el fabricante de reactivos, y su correlación con el método de referencia (ultra centrifugación), además que el costo elevado del reactivo crea incertidumbre para los responsables de los laboratorios clínicos sobre el beneficio real y necesario de cambiar la estimación del LDL-C por su determinación directa.

Abstract


The measurement of low-density lipoprotein cholesterol (LDL-C) is important in the routine clinical laboratory, since international guidelines for the management of patients with cardiovascular risk emphasize its use as a diagnostic classification criteria, and also its reduction as a therapeutic objective of treatment with statins or similars.

Nowadays, the measurement of LDL-C in most clinical laboratories in Costa Rica is carried out by estimation with the Friedewald formula, published in 1972, which is still widely used despite its limitations. So, the need to implement direct homogeneous methods for the determination of LDL-C is suggested.

The implementation of direct methods for the measurement of LDL cholesterol has been increasing in recent years; however, some doubts remain about the heterogeneity of the results depending on the reagent manufacturer, and its correlation with the reference method (ultra centrifugation ), in addition to the fact that  the high cost of reagent creates uncertainty for managers of clinical laboratories about the real and necessary benefit of changing from LDL-C estimation to the direct quantification.
